Minutes of the Second Organizational meeting

Rise Above Wellness Inc. The Second meeting Wellsville NY by founder Jeremy
Dahlgren at 11:35 am on December 21, 2019
Rise Above Wellness Center Inc. (Rise Above)

Those present were Jeremy Dahlgren, Elizabeth Freeland, Christina Petito and
Frederick Sinclair.

All present were invited to be members of the Board of Directors for Rise
Above and by consensus the officers were established as:
President: Jeremy Dahlgren,
Vice President: Frederick Sinclair,
Secretary, Elizabeth Freeland, and
Treasurer: Jeremy Dahlgren.

Adoption of Bylaws was appointed to be discussed to each officer before
placement of votes and record any changes if need be, on January 4th, 2020.

I Make a motion to authorize a corporate bank account was
opened through Stueben Trust as of December 21, 2019 remains 500.00$
established by Treasurer: Jeremy Dahlgren all said I.

1023 form applied with 501.c.3 tax exempt organization filed. Waiting on
response.

The status of the adjacent Building, Christina Petito met with Jeremy
Dahlgren and Elizabeth Freeland.

Christina's concerns are the structure and what its actual worth value.
159 N Main Street, Wellsville NY, 14895 is the address of adjacent building.

Jeremy Dahlgren asked Walkers Business for officer insurance quote, waiting
on response.

I make motion (Fredrick Sinclair) to authorize Jeremy Dahlgren to apply for
Wellsville Revolving loan If we should apply all said I.

Jeremy Dahlgren made motion to offer a personal loan to the Organization of
$17,000 dollar with zero equity. All said I. Christina Petito and Fredrick
Sinclair offered to work together with Alfred Technical Wellsville Campus.

All in favor of utilizing Rise Above Wellness Center Inc.
All in favor of working towards preparing the application and writing a
cover letter said I.

Fredrick Sinclair will help coordinate the environmental hazard evaluation.

Elizabeth Freeland will make arrangement's to meet all Practitioners for
2020 plans and what we can do to improve our organization. By stander Sheila
Kalkbrenner suggested we offer internship or community service.  All in
motion for an elevator, said I.

The next meeting was discussed and ended up being scheduled for Saturday,
January 4th at 11:00 am at the Rise Above Wellness Center.

This meeting adjourned at 1:20pm
